What insurance do you accept?

We accept most insurances. Your insurance will be verified prior to your initial visit. You will be contacted before your first appointment if we do not accept your coverage. It is your responsibility to inform us of any change in your coverage before any and all appointments. You always have the option of paying out of pocket at the time of service.

What if I have no insurance, can I still be seen?

Of course, but services will need to be paid at the time of your visit.

Do I need a referral to see one of your doctors?

This depends on the insurance coverage you have. We are considered a specialty service. Most carriers require you to have a referral. The decision is solely up to your insurance carrier as well as the coverage you have chosen. It is the patient's responsibility to obtain a referral should one be needed.

If I need a referral but fail to obtain one before my visit, will I be seen.

You must have your referral before you can be seen in our office. If you do not have your referral at the time of your visit, your options are:

  • Have your doctor's office FAX it to us at the time of your appointment.
  • Reschedule your appointment.
  • Pay for your services at the time of your visit.
Do I need to pay my co-payment or any outstanding account balance before I can be seen?

Yes. Co-payments are required to be paid at the time of your visit. Any outstanding balances on your account must also be settled before you can be seen.

What if I applied for insurance but have not been approved before my scheduled visit?

Insurance coverage is verified before your visit. If at the time of your scheduled appointment you do not have acceptable coverage, you will be responsible for payment before you are seen.

What forms of payment do you accept?

We accept cash, checks, money orders and most credit cards.
